GETTING STARTED AT PHYSIOSAGE

When booking your first appointment, you may be asked to add a credit card to your profile. Your card will not be charged unless necessary. We simply keep a card on file for late cancellations, missed appointments/no-shows, or outstanding balances.

If there is a remaining balance, our team will always ask first for permission before charging the card on file. Other payment options are also available.


INTAKE FORMS & CONSENTS

After booking your initial appointment, you will automatically receive intake and consent forms by email. These forms contain important health information needed by your practitioner and must be completed at least 24 hours before your appointment.

If forms remain incomplete, you may receive reminders from our admin team. If they are still not completed upon arrival, you must fill them out before entering the treatment room. Please note that your appointment will still end at the scheduled time, so time used for forms will reduce your treatment time.


BOOKING FOR YOUR CHILD

Please create a separate Jane profile using your child’s name and date of birth. The username must be different from yours and does not need to be an email address. Parent and child profiles can also be linked together. Our admin team can assist if needed.


DIRECT BILLING

We offer direct billing for most services through major insurance providers including Canada Life, Sun Life, Manulife, GreenShield, and Blue Cross. Please ensure your insurance information is completed in your intake forms before your appointment.


PARKING & LOCATION

Free parking is available directly in front of the building. Physiosage is located on the second floor. You may use the accessibility ramp on the left side of the building, once upstairs, we are the first room on the left or you can access the stairs through the first-floor entrance.

Physiotherapist with over five years of combined clinical, public health, and policy experience. Frances holds a Doctor of Physical Therapy (DPT) degree, along with Canadian certifications in Gerontology and Health Systems Management.

Her expertise spans musculoskeletal, orthopedic, neurological, and geriatric rehabilitation, delivered with a strong commitment to evidence-based, patient-centered care.
